Diagnosis of Parkinson’s Disease or experiencing Parkinson’s-like symptoms after being exposed to Paraquat AND:

• Paraquat exposure occurred in 1964 or later.

Strict Criteria

Diagnosis of Parkinson’s Disease or experiencing Parkinson’s-like symptoms after being exposed to Paraquat AND:

• Paraquat exposure occurred after 1961.
• Was not exposed to Agent Orange.
• If not used on a crop, the Paraquat must have been used on government-owned land.

Wide Criteria

Used a recalled Philips/Respironics CPAP or BiPAP machine and suffered one or more of the following ailments:

• Respiratory (Lung damage, New or worsening asthma, Pneumonia, Respiratory failure (such as Acute Respiratory Distress Syndrome), Pleural effusion, Reactive Airway Disease
• Cancer (Acute Myeloid Leukemia, Blood Cancer, Bone Marrow Cancer, Esophageal Cancer, Hematopoietic Cancer, Laryngeal Cancer, Leukemia, Lymphoma, Multiple Myeloma, Nasal Cancer, NHL, Soft Palate Cancer, Sinus Cancer, Throat Cancer, Tonsil Cancer, Thyroid Cancer, Papillary Cancer, Kidney Cancer, Liver Cancer, Lung Cancer, Bladder Cancer
• Kidney Damage (Acute kidney injury or chronic kidney disease)
• Liver Damage (Acute liver failure or chronic liver disease)
• Severe ear, eye, nose, throat, sinus, oral cavity inflammation and injury including nodules, cysts, and tumors
• Sarcoidosis (particularly of the lungs and/or lymph nodes that required treatment)
• Prospect is not a current smoker and must not have more than 15 pk/year smoking history (with 1 pk/year defined as smoking 1 pack per day for 1 year).

Strict Criteria

Used a recalled CPAP or BiPAP device for at least one year and subsequently been diagnosed with one of the following:

• ARDS, Chemical poisoning resulting in hospitalization, heart attack, heart failure, hematopoietic cancer, kidney cancer, kidney/renal damage/failure/AKI, leukemia, liver cancer, liver failure, lung cancer, lung disease, lymphatic cancer, multiple myeloma, nasal cancer, NHL, pleural effusion, RAD, respiratory failure, stomach cancer, asthma (only if newly developed condition after CPAP use and was hospitalized for Asthma.) Cannot be a current smoker at time of injury. Must have quit smoking for 5 years prior to injury

Wide Criteria

Used J&J Talcum Baby Powder or Shower to Shower usage for at least 4 years and was diagnosed with one of the following cancers:

• Ovarian
• Endometrial/Uterine
• Cervical
• Fallopian
• Vulvar
• Leiomyosarcoma

Strict Criteria

Used J&J Talcum Powder or Shower to Shower and was diagnosed in 2006 or later with one of the following cancers:

• Ovarian (no history prior to Talc use)
• Fallopian
• Peritoneal

If deceased, death occurred in 2010 or later.

Wide Criteria

Premature infant was fed cow-milk based formula (okay if unsure of brand) and was diagnosed with NEC or one of the following:

• Intestinal Perforation
• Sepsis
• Gastrointestinal/Bowel Problems
• Cerebral Palsy
• Neurological Conditions
• Other serious/lasting injuries

Strict Criteria

Infant was fed cow-milk based formula (okay if unsure of brand) AND:

• Was diagnosed with NEC
• If not premature, infant had surgery for NEC diagnosis

Wide Criteria

Hernia mesh surgery from 2000-current; Suffered from one of the following:

• Perforation
• Bowel Obstruction
• Mesh Erosion
• Mesh Contraction
• Torn Mesh
• Sepsis/Infection
• Granuloma
• Adhesion
• Dehiscence that resulted in a removal/repair, scheduled or future removal/repair

If injured party is deceased, the death occurred within the last 3 years, and cause of death is related.

Wide Criteria

Took Zantac or Ranitidine for at least 3 months; Diagnosed with Prostate, Pancreatic, Esophageal, Stomach/Gastric, Breast, Liver, Bladder, Colorectal, Intestine, Kidney, or Thyroid cancers between 2000 and today

Strict Criteria

Took Zantac for at least one year; diagnosed with Bladder, Pancreatic, Liver, Stomach, or Esophageal cancer (no history of smoking if Esophageal diagnosis)

Wide Criteria

• Had a knee or ankle replacement and the implanted device was a recalled product manufactured by Exactech.
• Knee replacement surgery took place on or after January 1, 2004.
• Ankle replacement surgery took place on or after January 1, 2017.

Wide Criteria

• Injured party resided, worked, or was otherwise exposed to the water supply at Camp Lejeune (including in utero exposure) for 30 days or more (consecutive or nonconsecutive); AND
• Exposure occurred between 1953 and 1987; AND
• Injured party was diagnosed with one or more of the following injuries after exposure:
  • Cancers:
    • Bladder cancer
    • Breast cancer
    • Esophageal cancer
    • Kidney cancer
    • Leukemia
    • Liver cancer
    • Lung cancer
    • Multiple myeloma
    • Non-Hodgkin’s lymphoma
  • Female infertility
  • Hepatic steatosis
  • Miscarriage
  • Myelodysplastic syndromes
  • Neurobehavioral effects
  • Parkinson’s disease
  • Renal toxicity
  • Scleroderma

Wide Criteria

Took Tylenol, Excedrin, DayQuil or NyQuil while pregnant. Tylenol, Acetaminophen, or Paracetamol must have been taken before and during the third trimester of pregnancy, AND child diagnosed with one of the following:

• Autism
• ASD
• Asperger’s
• PDD-NOS
• Childhood Disintegrative Disorder
• ADHD

If child is currently under 18 years old, mother must be legal guardian/pursuing claim on injured party’s behalf.

• If no, must be younger than 29 & mother must be willing to assist with the case.

Wide Criteria

Had gastric intestinal surgery where a surgical stapler was used internally AND suffered from one of the following after the surgical stapler malfunctioned or misfired:

• • Catastrophic Bleeding
  • Organ/Tissue Tear
  • Bowel Leakage
  • Fistula Formation
  • Subsequent Surgeries
  • Anastomotic Leak
  • Death
  • Colostomy
